How to File a Title II Compliant

This is in response to your request for information on how to file a complaint under title II of the Americans with Disabilities Act.

Title II prohibits discrimination based on disability in all programs, activities, and services of State and local governments.

If you feel you or another person have been discriminated against by any agency, organization, or institution covered by title II, send a letter to the Department of Justice, at the address below, including the following information:

{SYMBOL 183 \f "Symbol"} Your full name, address, and telephone number, and the name of the party discriminated against;

{SYMBOL 183 \f "Symbol"} The name of the agency, organization, or institution that you believe has discriminated;

{SYMBOL 183 \f "Symbol"} A description of the act or acts of discrimination, the date or dates or the discriminatory acts, and the name or names of the individuals who you believe discriminated; and

{SYMBOL 183 \f "Symbol"} Other information that you believe necessary to support your complaint. Do not send original documents. (Retain them.)
