Core city (Nagoya Citycomprising 16 wards) ■Population: about 2.2 million	→	■Outreach: First, go to meet with the isolated person, and give full personal support in solving problems and meeting the needs of users, thereby establishing a trusting relationship. ■Linkages: Acting as interpreters between users and the community has yielded better relations that ensure better access to the social resources the users need.	→	■Sources of Revenue (proportion): Subsidies: 70% Membership dues: 20% Contribution: 10% ■Networks: ・Users ・Volunteers ・Support organizations ・Local governments ・Local communities	→	■Community: The personal support system has been utilized through the collaboration of support organizations that have the expertise and the volunteers. The effects have spread over the area, causing the local government to implement this system as a municipal policy covering the city area. ■Users (families): 90% of persons in a state of total isolation have developed linkages with others. Many of them have started helping others as well.
