Application Procedure for Multimedia DAISY Textbooks Delivery Service
How to Request for Multimedia DAISY Textbooks 2015
All applicants requesting for multimedia DAISY textbooks 2015 must print and fill out application form, sign and send it by fax or mail.
In case of online application, after submitting information, applicants are also required to print the PDF file of the application form, sign and send it by mail.
Pupils and students with difficulties in reading ordinary textbooks can use multimedia DAISY textbooks. Medical diagnosis is not necessarily required.
Persons who may apply include parents, class teachers, resource room teachers, principals, board of education, supporters, pupils and students with print disabilities.

<Important Notices>
- Enable JavaScript to download application form.
- Adobe Acrobat Reader is required to print “Written Consent.”
<Notes when Completing the Form>
- If the pupils/students using multimedia DAISY textbooks belong to different schools, application should be submitted school by school.
- Make sure to enter the number of pupils/students using each of the multimedia DAISY textbooks.
<Additional Note>
- Applicants requesting for “Download from Server” will receive e-mail from JSRPD within a week. If not, please contact JSRPD.
- After completing online application form, click “Submit Application.”
- A screen with the title “Print PDF File of Application Form” will appear. Click “PDF File for Printing Application Form” and print the document.
- Fill in “Affiliation” and “Signature” and fax/mail to JSRPD.
- After checking your application form, JSRPD will send you an e-mail explaining how to download multimedia DAISY textbooks. Please acknowledge our mail.
If you request for “Receive CD by Mail,” please fax the printed application form to JSRPD in the same manner.
Please contact JSRPD if you don’t receive our e-mail even after a week.
<Important Notice>
If you request for “Receive CD by Mail,” please note that “2,000 Yen to each student for each subject” will be charged.
